Adult children as paid caregivers in Fresh Meadows single-family homes

Fresh Meadows is a neighborhood of detached and semi-detached houses between the Long Island Expressway and Union Turnpike, and the most common home care story here is a parent who has moved into an adult child's house, or never left the family home while the child moved back in. The Consumer Directed Personal Assistance Program (CDPAP) can pay that adult child as the parent's personal assistant. Agency personal care services (PCS) would instead send an aide employed by a home care agency. This page is about that choice, the rules on relatives, and the money question that comes up in a neighborhood of homeowners on fixed incomes.

Medicaid for Fresh Meadows residents is run citywide by HRA. The HRA Medicaid Helpline, 1-888-692-6116, handles eligibility, renewals, and surplus income budgets. Fee-for-service personal care and CDPAP cases go through HRA's Home Care Services Program at the DSS OneNumber, 718-557-1399. If the parent is in a Managed Long Term Care plan, the plan authorizes hours, and New York Medicaid Choice at 1-800-505-5678 (TTY 1-888-329-1541) answers plan questions. 1. The parent's Medicaid is active, or the excess income question is worked out first, and a home care assessment finds a need. Since September 1, 2025 the state's Minimum Needs Requirements apply to new assessments of people 21 and older; those already receiving services continue under the earlier criteria, per the Department of Health.
2. The plan or HRA issues an authorization. CDPAP usually needs its own authorization and person-centered service plan, distinct from agency care.
3. The parent, as consumer, chooses the personal assistant. The Department of Health excludes the spouse, the designated representative, and the parent of a consumer under 21. An adult son or daughter, including one who lives in the same house because the parent's care makes that presence necessary, generally may serve under the state's rules.
4. Both register with Public Partnerships LLC (PPL), the statewide fiscal intermediary designated by the Department of Health: 1-833-247-5346, TTY 1-833-204-9042, through PPL@Home online, a facilitator, or an in-office appointment (the DOH consumer resources sheet lists a Forest Hills office). Check health.ny.gov/cdpap for current options.
5. The adult child completes the state-required health assessment before service begins, then records hours through PPL's timekeeping; the parent, or the designated representative if the parent cannot self-direct, approves the time.

Many Fresh Meadows homeowners have pension and Social Security income above the Medicaid limit. The Department of Health's excess income program lets a person still get Medicaid, including home care, by meeting a monthly surplus with medical bills or the pay-in option. For a person certified disabled, income placed in a pooled income trust run by a non-profit is not counted, once HRA has a copy of the trust and a written statement of the monthly deposit. • Assuming a daughter who lives in the house cannot be paid because she "already lives there." The state's rule allows a co-resident relative when the amount of care makes that relative's presence necessary; the hard exclusions are spouse, designated representative, and parent of a consumer under 21.
• Making the same daughter the parent's designated representative on the forms. A designated representative cannot also be the personal assistant, so if the parent cannot self-direct, a different relative should hold that role.
• Selling or retitling the house before asking HRA how it is treated. Nothing on this page is legal advice; ask HRA and a counselor before moving any asset.

Managed Long Term Care members appeal to the plan first, within 10 days of the notice date or effective date to keep hours unchanged; after a final adverse determination there are 120 days to request a fair hearing but 10 days to keep aid continuing.
